OR1 Questionnaire to Identify Participation in Extended "Pick-and-Go" Breakfast and Lunch Program
Launch the media gallery 1 playerUpon State approval, District OR1 will extend its breakfast and lunch program to include "Pick-and-Go" meals. Students’ cost for the meals and free/reduced eligibility will not change from the school year. However, families that were “full pay” during the year that have a change in income are strongly encouraged contact your building secretary to apply for free/reduced eligibility. Please note, participating families may choose the days and meals they wish to receive.
First, we must get a sense of how many families intend to utilize this food service. The link at the bottom of the page connects you to a survey asking if your family will participate under two different scenarios. Other than demographic questions, the first question will ask if you would participate in the "Pick-and-Go" breakfast and lunch program if we arrange “pickup” points at the Bennet and Palmyra schools. The next question will ask if you would participate in the "Pick-and-Go" breakfast and lunch program if we used identified locations in Bennet, Palmyra, and Douglas for pickup points. The final question will ask for any feedback you may have for this process.
